If you find it (looks like the picture above), head over here, enter your unique 16 digit code that is listed on the bottom of your receipt, complete the survey and upon completion, you’ll receive a printable $3/$15 coupon that is good for 2 weeks! Note that it appears you can only earn a maximum of three $3/$15 coupons per month.
